Password Initialization Application Form

パスワードの初期化は職員が1件ずつ行うため、初期化完了までに時間をいただくことをあらかじめご了承ください。
フォームによる申請は常時受け付けておりますが、初期化作業は平日9:00～16:00に実施いたします。

Please note that it may take some time for this process as it is done one by one by our staff.
We are always accepting request, but the initialization process will be done between 9:00 and 16:00 on weekdays.

パスワード初期化申請前に、以下のWEBサイトに掲載の「ログインできないときは」を参照してください。
ログインできない場合に表示されるエラー画面の例を確認することができます。

特に、期限までに2段階認証を設定しなかったユーザーがパスワード初期化申請を行うケースが散見されますので、ご注意ください。2段階認証設定やアカウントロック解除申請については、以下のWEBサイトを確認してください。

申請前の確認事項①

ご自身私用のGoogleアカウント（～@gmail.com）持っており、ブラウザにアカウントやパスワード情報を記憶させていると、うまくログインできないことがあります。
If you have a private Google account (~@gmail.com) and your browser remembers your account and password information, you may have trouble logging in.

そのため、以下の方法により、一度すべてのGoogleアカウントからログアウトしてください。
Therefore, please log out from all Google accounts once by following the instructions below.
